ABOUT WILDERNESS MEDICINE

By definition ‘Wilderness’ in the context of ‘Wilderness Medicine’ means when one is at least an hour away from medical help, which effectively in the Indian scenario applies to almost every place, let alone the remote & wild places. “Wilderness Medicine means standard medical principles provided in a context that requires attention to extended contact time with the patient, environmental extremes, treatment with limited and non-specialised equipment that may require improvisation, and the possible lack of communication.”

CURRICULUM

Full Patient Assessment & Extended Care; Injury-Illness Identification & Management including: Bleeding Control, Shock, Spinal Injuries, Head Injuries, Chest Wounds, Sprains & Strains, Fractures & Dislocations, Splinting, Cold Injuries, Heat Illnesses, Lightning, Altitude Illnesses, Drowning, Anaphylaxis, Bites & Stings, Medical Illnesses, Wounds & Wound Management, Patient moves & Litter carries, Asthma, Diabetes, Abdominal Emergencies, Burns, Environmental Emergencies; Cardio-Pulmonary Resuscitation (CPR) including MI, Angina & Stroke; Emergency Response Procedures, etc.

CERTIFICATION

If a participant is able to meet the expectations set out by WFA course then that participant gets a ‘Wilderness First Aid’ certificate which has a validity of two years. Ditto for CPR, which invites a distinct certificate. Re-certification for both involves undertaking the full course again after two years. If a participant does not meet the expectations set out by WFA course then that participant gets a ‘Wilderness Medicine Essentials’ (WME) certificate with a validity of two years. WME is an 8-hour course. All participants are informed about this at the beginning of the course. We have enough time during the CPR session to coach and mentor a participant to enable him/her to meet the requisite expectations.

ACCREDITATION

Credibility of these certificates: Hanifl Centre of Woodstock School, Mussoorie has collaborated with Aerie Backcountry Medicine (ABM), Montana, U.S.A. to teach these courses in India. ABM has worked with other organisations in the USA specialising in wilderness first aid (e.g., WMI and WMA), and under the guidance of the Wilderness Medicine Society, evolved a Scope of Practice (SoP) for courses like the WFA. So not only is its content vetted but there is a review system in place where this SoP stands to be modified from time to time. ABM trained instructors through a 10-day ‘instructor’s course’ – all students on that course were already qualified in a 80-hour Wilderness First Responder course and have had field experience in the outdoors. The participants get our Wilderness Medicine manual besides the 2 cards. These cards are accredited by Wilderness Medicine Society (WMS) and American Heart Association (AHA).

FORMAT

As there will be a lot of scenarios and some participants will be acting as patients while others become care providers, loose fitting outdoor clothes would be better. Also we use fake blood and moulage (which are food grade vegetable colours and easily washable) so dress accordingly.

About 40% of this course would be ‘practicals’. All topics will NOT have a practical. We shall elaborate on this during the course; but suffice it to state here that the WFA comes with a clearly defined ‘thrust’ and expectations from participants, and some topics get covered ‘theoretically’ – the SoP mentioned above guides this composition and delivery.
